How to Submit an Instructor Application
Instructor Application:
- Go to the MyProfile login page at https://dice.fldfs.com/public/pb_index.aspx.
- Select Create Account.
- Select Instructor on the drop-down menu.
- Enter current instructor ID and Date of Birth, if applicable, or select New Instructor Application.
- Follow screen prompts to complete the application.
- Submit the application.
Note:
Instructors must meet the requirements listed in either
69B-228.060 FAC for Continuing Education or 69B-227.160 FAC for Prelicensing.
Non-US citizen applicants must provide the following:
Copy of social security card AND
Copy of permanent resident card OR
Copy of work authorization card.
