October 22, 1996. All policies with FESA were cancelled effective November 25, 1996. Proof of claim forms have been mailed.

The First Interim Report on Claims was filed with the Second Judicial Circuit Court in and for Leon County,
Florida (“Court”) in the estate of Florida Employers Safety Association Self-Insurers Fund (“FESA”). The Court issued an order
approving the report on June 1, 2007. On June 6, 2007, notices were mailed to 3,258 claimants who filed a proof of claim in the
FESA estate. The notice informed them of the Receiver’s recommendations concerning the classification and amount of their claim,
along with instructions on how to proceed if a claimant objected to the recommendations. Objections were required to be filed by
the claimant, in writing, with both the Court and the Receiver and postmarked by July 16, 2007. Objections filed after that date
were not considered.
On February 26, 2008, the Court approved the Final Claims Report regarding the distribution of assets in this
estate. Under the terms of the Order, Class 1 claimants received a 100% distribution and Class 2 claimants received a 83.0402%
distribution of the adjudicated amounts on their claims. Distributions were mailed on March 25, 2008. No additional distributions
were made in this receivership.
On June 26, 2009, the Court issued an Order that discharges the Florida Department of Financial Services of
it duties as Receiver of FESA effective June 29, 2009.
