How do I obtain a proof of claim form?

The Receiver has mailed proof of claim forms. If you have not received a proof of claim form, please request a form using our "Contact Us" form. Alternately, you may contact the Receiver at 850-413-3081
.

Will my commissions (Agents) or General Creditor claims be paid by FIGA?

No. Agents and general creditor claims are not covered by FIGA, but may be raised as claims in the receivership estate. There may not be sufficient assets to pay this priority of claims; however, you must file a Receiver's proof of claim form to have your claims considered.

If I began the regular claim proceedings with Fortune, will I still need to submit a proof of claim form?

Yes. The Receiver has mailed out proof of claim forms. In order for any claims to be considered "timely filed", you must have completed the form and returned it to the Receiver by the filing deadline of October 4, 2002.
