If the caller is inquiring about a new claim or a pending claim for services rendered on or after July 20, 2007, please refer the caller to United Healthcare of Florida at 1-888-507-9352. This company took over the policies previously provided by AHC. United Healthcare of Florida is providing coverage under their Secure Horizons Plan.
Skilled Nursing Care Claims
