Pending Loss Claims – Services Rendered on or After July 20, 2007
If the caller is inquiring about a new claim or a pending claim for services rendered on or after July 20, 2007, please refer the caller to United Healthcare of Florida at 1-888-507-9352. This company took over the policies previously provided by AHC. United Healthcare of Florida is providing coverage under their Secure Horizons Plan.

Skilled Nursing Care Claims
If the caller is inquiring about a claim that involves skilled nursing care under a program called Senior’s Choice, please refer the caller to Humana at 1-800-594-8379.
