APPLICATION FOR BOARD APPOINTMENT — APPLICATION WINDOW RE-OPENING BRIEFLY The Department has briefly re-opened the window for applications for appointment to the Board of Funeral, Cemetery, and Consumer Services. The appointment of three current members of the Board of Funeral, Cemetery, and Consumer Services will be expiring in the coming months. To apply, submit a Main Application Questionnaire, and a Supplemental Questionnaire, in time to be actually received by this Division BY NOT LATER THAN Sept. 9, 2011 (Posted 8-5-2011)BODIES HANDLED REPORTS — Concept for electronic submission via CSV files The FCCS Division is considering ways to streamline the submission of monthly "bodies handled" reports. Persons or firms who provide or use software that does or might allow export of bodies handled data to a CSV file for forwarding to the Division, are asked to read the attached item. Posted 2-5-09 LICENSED ENTITIES – A CHANGE IN BUSINESS LOCATION, OWNERSHIP OR A SUBSTANTIAL MODIFICATION TO THE PHYSICAL PREMISES OF A LICENSED ENTITY A change of actual physical location, a change in ownership or a substantial modification to the physical premises of a licensed funeral establishment, cinerator facility, direct disposal establishment or centralized embalming facility requires Board approval. The licensed entity is required to submit the change for Board approval using the appropriate entity license type application form. NEW E-MAIL FORMAT Effective immediately please use the following e-mail format when contacting the Division of Funeral, Cemetery and Consumer Services staff: FirstName.LastName@myfloridacfo.com IMPORTANT NOTICE TO LICENSE APPLICANTS Be sure to allow adequate time for your license application to be reviewed. Chapter 497.167(9), Florida Statutes, requires an application to be complete at least 25 days in advance of a regularly scheduled board meeting to be considered by the board at such meeting. The Licensing Unit has internal deadlines for identifying application deficiencies, processing completed applications and submitting applications for board action. Application processing begins when an application is received in the Licensing Unit. All requirements must be received, reviewed and acted upon two weeks prior to the next board meeting. Board agendas are then produced and all processed applications and documents are copied and submitted to the board members for their review one week prior to the board meeting. CONTINUING EDUCATION PROVIDERS – COURSE ROSTERS The Division uses the DICE electronic data tracking system to manage continuing education data. This system has been enhanced to allow course providers to enter course completion information directly into DICE, from course rosters, by using the attendee’s social security number or FCCS license number. |
Andrew Clark (Ocala, Florida) was appointed to serve on the Board of Funeral, Cemetery and Consumer Services. He was appointed to serve a term beginning December 2, 2011, and ending September 30, 2015. He succeeds Tracy Huggins On October 5, 2011, it was announced that Richard Mueller (Jacksonville, Florida) was appointed to serve on the Board of Funeral, Cemetery and Consumer Services. He was appointed to serve a term beginning October 6, 2011, and ending September 30, 2015. He succeeds Virginia Taylor. During the October Board meeting, Mr. Mueller was in appearance at the meeting as part of the audience, and recognized by the board for his recent appointment.
