TALLAHASSEE -- Florida's State Insurance Commissioner issued an alert for three suspects being sought on state charges of fabricating health insurance claims worth an estimated $10 million.

Facing prior federal charges for drug trafficking, the three Miami men are believed to have fled to Bogota, Colombia.

Last month, a Statewide Grand Jury focusing on insurance fraud indicted them and a fourth Miamian on charges of insurance fraud, grand theft and racketeering. State Division of Insurance Fraud investigators on Feb. 24 arrested Nelson Vallin, 53, a licensed insurance agent.

The three suspects still at large are Alex Neyman, 51, and his sons, Samuel Hernandez, 29, and Luis Hernandez, 26. Although investigators believe the suspects may have fled the U.S. for Colombia, Luis Hernandez was seen as recently as last month at a Denny's Restaurant in Southwest Miami. Neyman was last seen in the Miami area in January 1997, and Samuel Hernandez was seen there in February.

How the scheme works: Fraud artists target both patients, doctors and companies alike. They gain access to patient histories and doctors' identification numbers, and then submit fabricated claims to insurance companies for procedures and tests on patients who may never have had any health problems at all. The claims checks are then mailed to the perpetrators' rented post office boxes, retrieved by a runner, and cashed.

One facet of this problem, is patients who may never have had health problems before could have a false health history created for them in this type of insurance crime. As a result, they could be refused insurance in the future if they change jobs or policies.

All told, medical claims fraud nets insurance fraud criminals about two thirds of the staggering $6.9 billion per year it costs Florida. This insurance crime alone costs each Florida family more than $1,000 per year in higher premiums.

Information was received from numerous insurance companies who received bills for medical treatment that their insureds never received. It was later determined that Alex Neyman, Samuel Hernandez, Luis

Hernandez, and Nelson Vallin were involved in collecting the proceeds from fraudulent medical insurance billing using the names of approximately one hundred and sixty (160) medical providers that did not actually exist. Furthermore, Neyman, Samuel Hernandez, and Luis Hernandez, oftentimes using aliases, obtained major medical insurance for eight shell corporations, and then subsequently billed fraudulently on behalf of the alleged employees.

Investigation by the Florida Division of Insurance Fraud has revealed that approximately nineteen (19) insurance companies received health insurance claims made through the United States Postal Service in 1996 and early 1997, totaling almost $5 million dollars. Of these claims, over $725,000 have been paid to the purported medical providers. The mailing addresses listed on the medical claim forms for these providers were all mail drop facilities, which were rented by Neyman or Samuel Hernandez, using several different aliases.

Nelson Vallin was arrested on February 24, 1998. Active arrest warrants were obtained for the other three subjects, who are at large. The charges include Racketeering, Conspiracy to Commit Racketeering, numerous counts of Grand Theft First Degree, Second Degree, Third Degree and Insurance Fraud.

Luis Hernandez was seen as recently as February, 1998 at a Denny's Restaurant in Southwest Miami. Neyman was last seen in the Miami area in January 1997, and Samuel Hernandez was seen there in February, 1997.
