Education Providers:
How to Submit an Application to become an Education Course Provider

1. Go to MyProfile at https://dice.fldfs.com/public/pb_index.aspx.
2. Select "Create Account".
3. Select "Provider" on the dropdown menu.
4. Enter current School Official ID and Date of Birth, if applicable, or select "New Provider Application".
5. Follow screen prompts to complete the application.
6. Submit the application.
7. Submit an electronic copy of your resume to Education@MyFloridaCFO.com with your name in the subject line.
