Application Submittal Instructions
- Go to
MyProfile to log in.
- Click on the blue “Provider” tab to enter your social security number and
date of birth. Click "next."
- Select "Continuing Education" or "Pre-licensing."
- A drop down box will open with several course authority selections, Example
"220-Property and Casualty." Select one, then click "next."
- Enter all information for required fields (red.) Complete background
questions (after each question click "next").
- For the school official application, enter all information for required
fields (red).
- Complete background questions. After each question click "next." When you
are finished, click "submit."
- Click "add" if your request is to have the school official as a contact.
Enter the required information and click "submit."
