COMPTROLLER'S MEMORANDUM NO. 25 (1995-96)
SUBJECT: CASH AWARD FORMS FOR MERITORIOUS SERVICE AWARDS PROGRAM
Chapter 60L-16, Florida Administrative
Code, was repealed effective April 4, 1996 and replaced by the
Department of Management Services through its issuing of new
Guidelines and Procedures for the Meritorious Service Awards Program.
Pursuant to these new Guidelines and Procedures all agencies
are advised to use the attached revised forms, DBF-BP-38 and DBF-BP-39
as appropriate when requesting payment for Meritorious Service
Awards. Please duplicate as needed as we will not be maintaining
a supply of these forms.
