AGENCY 5% TRANSFERS

Historically, agency 5% transfer requests were sent directly by state agencies to the Department of Financial Services for processing.  Effective July 1, 2005, agencies were asked to submit all 5% transfer requests utilizing LAS/PBS Form 6 and conform to the existing budget amendment process.  State agencies will be expected to submit a paper copy of Form 6 and an electronic copy directly to :
