Direct Deposit (Electronic Funds Transfer)
Authorization Forms |
The Comptroller's Office is continuing modernization efforts of business
processes. We will be converting to computer imaging technology from
microfilm for long-term retention of document images. The first application
of this new process will be the Employee-Retiree Direct Deposit (EFT)
Authorization Form. Uniformity of size and design will assist in this
process and as such, a Microsoft Word file of the new form is attached
to the e-mail distributing this memorandum. All previous versions of
the form are obsolete and should be recycled. Please ensure transition
to the new form is complete no later than May 25, 2001.
Employees who receive warrants are encouraged to use the form printed
on the back of the employee earnings statement.
The initial supply of forms may be obtained by sending e-mail to
directdeposit@mail.dbf.state.fl.us.
Please specify the number of forms needed and the mailing address. Departments
and agencies with numerous field offices are requested to consolidate
orders and perform distribution. Re-supply will be accommodated in normal
fashion.
The new form is also available on the Comptroller's web site at
