FIREFIGHTER EMPLOYMENT AND TERMINATION

Upon the employment of a firefighter, the employing agency shall forward to the Bureau of Fire Standards and Training a completed  Notice of Employment (Form DFS-K4-1032) which evidences that the new employee is certified as a firefighter and has met the requirements necessary to maintain such certification as specified in State Statute 633.352. This form shall be forwarded within ten (10) business days after the first day of employment.

Upon the termination of employment of a firefighter, the employing agency shall forward to the Bureau of Fire Standards and Training a completed Notice of Termination (Form DFS-K4-1033). This form shall be forwarded within ten (10) business days after the date of termination.
