CLAIMS REPORTING
WORKER’S COMPENSATION
· Report all claims to OptaComp at 1-877-518-2583.
o In a medical emergency, transport the injured employee to the nearest medical facility or call 911 for emergency assistance.
o Following the arrangement of emergency medical treatment for the injured employee, contact OptaComp at 1-877-518-2583.
o If the injury does not require emergency treatment, contact OptaComp at 1-877-518-2583 immediately and prior to obtaining medical treatment.
o OptaComp will refer the injured employee to an appropriate medical provider from the OptaComp clinician panel. OptaComp provides this service 24 hours a day, seven days a week, 365 days a year.

If your claims
involves an automobile accident:

Get the names, addresses and telephone numbers of any
witnesses to the accident.
·
Report any accident immediately to your supervisor in
accordance with your agency’s procedures.
·
Don’t promise to pay anything.
·
At the scene of the accident, do not discuss details
of the accident with anyone except the
appropriate investigating law enforcement
officer.
·
Obtain information about the other driver (or
drivers) from the law enforcement officer.
If your claim
involves bodily injury to another or damage
to property:

Complete the
General Liability Loss Report Form and
forward it to Risk Management:
