1.  A Law Enforcement officer asks for my agency’s automobile policy number following an accident,  how do I determine my agency’s automobile policy number?

Each agency policy number is established using the following convention AL 00-0000:

AL -      auto liability.

00 -      the first two digits are the start of fiscal year (i.e. fiscal year 2002/2003 would be 02).

0000 - the second four digits is your agency location code (i.e. the Department of Transportation’s location code is 2600).  Therefore, the Department of  Transportation's automobile policy number for            FY 02/03 is AL 02-2600.

Your agency’s automobile policy number can be kept on the Fleet Automobile Liability Insurance Information pamphlet.

  2.  What should I do if I am involved in an automobile accident?

If you are at fault or partially at fault:

*Contact Risk Management immediately at 850-413-3122

*Have the accident investigated by a law enforcement officer.

*Complete the Automobile Accident Report Form and forward it to Risk Management at:

                        Department of Financial Services

                        Division of Risk Management

                        State Liability Claims

                        200 E. Gaines St.

                        Tallahassee, Florida 32399-0338

*Get the names, addresses and telephone numbers of any witnesses to the accident .

*Report any accident immediately to your supervisor in accordance with your agency’s procedures.

*Don’t promise to pay anything.

*At the scene of the accident, do not discuss details of the accident with anyone except the appropriate investigating law enforcement officer.

*Obtain information about the other driver (or drivers) from the law enforcement officer.

If you are not at fault:

*Report accident immediately to your supervisor.

*Do not report to Risk Management.

3.  A visitor in a state owned building just fell what should I do?  I am a state employee and I was working in the field and ran over a mailbox, what should I do?

*Complete the General Liability Loss Report Form and forward it to Risk Management:

 4.  If I’m sued for negligently performing my job duties does the Division of Risk Management cover me and what should I do?

Yes, you are covered.  Per state statute an employee of the state of Florida acting in their official capacity cannot be named individually in a lawsuit.

You must contact your agency’s General Counsel's office or District Legal Counsel’s office.

5.  What should I do I am no longer an employee of the state of Florida and I am being sued for negligently performing my job duties while I was an employee?

Contact your General Counsel’s office or District Legal Counsel’s office.
