If you are filing a claim against a company in receivership and any future distributions will be taxable to you or
your organization, you will need to have a completed IRS W-9 Form on file with the Receiver.
In order to submit a IRS W-9 Form, you will need to visit the Internal Revenue web site at
www.irs.gov and download the form and instructions.
The completed IRS W-9 Form should then be submitted along with your proof of claim submission.
