Prospective Vendor Information and Associated Forms

This web site presentation is intended to provide guidance to vendors that express interest in becoming a pre-approved services provider to the Florida Department of Financial Services, Division of Rehabilitation and Liquidation ("Receiver").

The information requested below, when submitted, will be used in support of your company or firm becoming an approved services provider to the Florida Department of Financial Services, Division of Rehabilitation and Liquidation.

All vendors must be approved before they may offer services to the Receiver directly, or via the Receiver’s contractors. Please note that this approval process is completely separate from any registration that may be required in MyFloridaMarketPlace for any other Department or Division within the State of Florida.

If the information submitted is approved by the Receiver, and there are any future Division project needs that closely match the expertise and proposed services offered by your company or firm, a representative of the Receiver will contact you (or your named designee) for more information at the time the services are needed- usually in the form of a Request for Proposal; or, in the alternative, via e-mail, telephone call, letter or by facsimile.

The designation of "approved vendor" does not guarantee or imply the assignment of any immediate or future project or request for services by the Receiver.

The Receiver uses a fair and open "competitive bidding" process for selecting or matching a vendor to a project ("competitive bidding" is a method of procurement by which two or more parties are given the opportunity to submit separate bids in response to specific guidelines or requests for services. "Fair" means that all bidders are treated the same and that no bidder has advance knowledge of the project information. "Open" means there are no secrets in the process – such as information shared with one bidder but not with others – and that all bidders know what is required of them).

There are two forms you may select from:

Division of Rehabilitation and Liquidation- Application for Contract Provider pdf 20kb (for non-legal services)

Division of Rehabilitation and Liquidation- Application for Legal Services Provider pdf 20kb (for legal services only)

Please select the form that most closely matches the expertise and type of service that your company or firm proposes to provide (legal or non-legal). The selected form must be completed in its entirety and submitted to us along with a cover letter. The cover letter should briefly outline the specific, proposed services your company or firm provides, and reference all required attachments you will submit with the Application.

The required attachments to be included with the Application are:

  1. A copy of the company or firm’s corporate or professional resume - you may submit a corporate brochure or booklet; print the information on the firm's website, or, if your company or firm does not have a corporate resume or brochure, please write a professionally edited narrative that contains specific detail(s) about the organization and the services it provides - this narrative should also reference those items that you will submit (resumes/qualifications/ licenses, etc).
  2. Copies of current resumes/qualifications and information on the professional license(s) of individual firm members who may provide services to the Receiver (Bar membership; actuarial/CPA/accounting certifications; adjuster licenses, etc.); and of probable professional and administrative-level personnel who may also provide services to the Receiver.
  3. A copy of the company or firm's Certificate of Liability Insurance, which should include current, up-to-date policy types, numbers, effective and expiration dates, and limits.

Once the below information is received, we will continue the process of approving your company or firm to become a pre-approved services provider to the Receiver:

  1. the completed Application for Contract Provider OR Application for Legal Services Provider form and cover letter
  2. the company or firm's corporate resume (which should include current information about the organization and its services)
  3. the resumes and qualifications of individuals who may potentially provide legal services or non-legal services to the Receiver)
  4. the firm's current, up-to-date Certificate of Liability Insurance

Please mail the completed packet to the address below. You may also submit your completed packet electronically via e-mail to Jeannette.Johnson@myfloridacfo.com. You will receive notification via e-mail if other items are needed to complete your packet or if your packet is complete and ready to be processed. An incomplete packet will not be submitted for approval processing.
