Insurance Carrier Registration

Each insurance carrier that is newly approved by the Florida Office of Insurance Regulation to write workers’ compensation insurance, Line of Business 160, must register with the Division of Workers’ Compensation and pay a registration fee of $100 before writing such insurance.

How To Register

  1. Verify Certificate Of Authority
    Insurers can not register with Division of Workers’ Compensation until they are approved by the Office of Insurance Regulation to write Line of Business 160 Workers’ Compensation.
  2. Register Notification
    The Division will issue a welcome letter with the username and password to the new insurer to begin the online registration, when a Certificate of Authority for a Line of Business 160 has been issued or added for a Insurer.
  3. Online Registration
    Once the Carrier gets their username and password from the Division, the Insurer will go to https://apps.fldfs.com/Survey/default.aspx?qid=87 to complete the online registration.
  4. Verify Submission
    When the new carrier has completed the online registration, please email carrierregistration@myfloridacfo.com to verify submission. Failure to send an email may result in a delay of assigning your carrier identification number and the authorization to write Workers' Compensation insurance in the State of Florida.
  5. Pay Registration Fee
    In order to properly complete the registration process, the $100 registration fee and invoice must be mailed to the Division at PO BOX 7300, Tallahassee, FL 32399-7300. At this time only checks are accepted.
  6. Assignment of Carrier ID Number
    Once the payment is received, a carrier identification number will be sent to the carrier.

How do I get a Carrier Number?

Once a carrier gets a Certificate of Authority the Division will issue a welcome letter with a username and password. The carrier will need to register online at https://apps.fldfs.com/Survey/default.aspx?qid=87 and send in the $100.00 registration fee. At that time the Division will issue a Carrier ID.

How do I know if I have been approved to write Workers’ Compensation (Line of Business 160)?
