In accordance with section 440.52, F.S., each insurance carrier who desires to write Workers' Compensation insurance in compliance with this chapter shall be required, before writing such insurance, to register with the Division and pay a registration fee of $100. The Registration Fee Invoice, available on the registration form page, is to be included with your payment and provided to the Division as outlined in the invoice instructions. After the registration fee has been paid and once the registration forms are electronically sent to the Division, a specific carrier identification number will be assigned to your company.
The Division of Worker's Compensation uses this registration information when assigning insurer identification numbers, tracking medical and indemnity claims, and to effectively communicate and disseminate penalty and Division information to newly registered insurers in a timely fashion.
In order to properly complete the registration process, the $100 registration fee must be mailed along with the registration fee invoice to the address listed on the invoice. In addition, the required registration forms, listed in Section I below, must be completed and electronically sent to the Division. After completing the required registration forms, new carriers must review, acknowledge and electronically verify that they have received the pertinent programmatic information regarding each bureau listed in Section II.
