Claims Information

How do I report a new claim or check on the status of a pending claim?

New claims must be reported in writing by fax, UPS/FedEx or regular mail only. The initial notice of claim should contain all pertinent supporting documentation. The supporting documents required will depend on the nature of the claim. The initial notice of claim should contain:

  • A completed Claim Submission Form. Click here for the form.
  • Supporting documentation including but not limited to:
    • Copy of the title insurance policy.
    • Copy of the final settlement statement (or HUD-1).
    • Any additional documents that the claimant wishes to have considered in support of the claim.

Claimants can submit a claim by:

  • Fax to (850) 413-3997, or
  • Mail or UPS/FedEx to:

Receiver for National Title
Attention: Claims Section
Division of Rehabilitation and Liquidation
2020 Capital Circle SE, Suite 310
Tallahassee, FL 32301

If I have already filed a claim with National Title, will the Rehabilitation affect the status of my claim?

The Receiver is currently transferring the administration of all claims to a new service provider for further handling and resolution. Your patience is appreciated.

Florida Policyholders: No. To the extent possible, claims will be processed in the ordinary course of business.

Alabama, Georgia and Tennessee Policyholders: The claims filing deadline was July 13, 2012.

How can I find out the status of my claim?

You may contact the Receiver by using the "Contact Us" form to send us an email if you have any questions or concerns about the status of your claim or the Rehabilitation process.
