If your policy has already cancelled and you have not received your refund, you should contact the Third Party Administrator (TPA) retained by FLAHIGA to provide administrative and claims processing services for AvaHealth at (847) 803-9629. Upon cancellation of policies by FLAHIGA, the guaranty association will refund unearned premiums.
If you believe you will incur out of pocket expenses as a result of the liquidation or otherwise believe you are owed monies by this company, you will need to complete a proof of claim form and file it with the Receiver. The Receiver has posted a proof of claim form on the Receiver’s website, www.myfloridacfo.com/Receiver, under Proof of Claim Form General Information.
