How do I report a new claim?

New claims must be reported in writing by fax, UPS/FedEx or regular mail only. The initial notice of claim should contain all pertinent supporting documentation. The supporting documents required will depend on the nature of the claim. The initial notice of claim should contain:

• A completed Claim Submission Form. Click here for the form.
• Supporting documentation including but not limited to:
• Copy of the title insurance policy.
• Copy of the final settlement statement (or HUD-1).
• Any additional documents that the claimant wishes to have considered in support of the claim.

If I have already filed a claim with KELTIG, will the Rehabilitation affect the status of my claim?
